Abstrakt:
Mixed payment systems for physicians combining fee-for-service and
capitation are advocated in the theoretical health economics literature
as optimal levels of quantity of medical services can be achieved.
Empirically, however, it is still not well understood how physicians’
service provision is affected by mixed payment systems. In particular,
it is not clear whether mixed systems perform better than traditional,
‘pure’ fee-for-service or capitation systems. In our laboratory
experiment, we systematically compare individual physicians’ provision
of medical services in traditional and mixed payment systems. Subjects,
in the role of physicians, decide on the quantity of medical services
for different types of patients in the laboratory. Real patients,
outside the lab, benefit from subjects’ decisions. We find that
overprovision under fee-for-service and underprovision under capitation
can, in fact, be reduced by mixed payment systems rendering a higher
health benefit to patients. Mixed payment systems also imply higher
health benefit‐remuneration ratios than the respective traditional
payment systems. Further, our findings provide some interesting insights
for reforms of physician payment systems.
